You are required to complete an online OHS induction before commencing work at Park Beach Plaza, Park Beach HomeBase and/or Gateway House. Please fill in the following information and we will email you a password and instructions on how to complete your induction online.
If you require additional information please make a note in the additional comments section or contact Centre Management on 02 6652 4366. Please note that a password cannot be issued until you supply the information below & your insurances are received.
Who should be inducted?
Contractors working for Retailers including trolley collectors
Couriers and anyone delivering goods into any Centre
Contractors working for Park Beach Plaza, Park Beach HomeBase or Gateway House
Documents that will be required:
Public Liability Insurance (minimum $20 million)
Workers Compensation Insurance (unless you are a sole trader)
Safe Work Method Statement specific to the site and job
Passport type JPEG photo file to upload for each induction card (Do not send this to the Plaza, you will need it when you do the online induction)
